We are the go to organisation providing support to carers and others, across the Coast and Vale. The Home from Hospital service is not just for those returning home alone from Hospital or for those who have had a long stay in hospital, but for any client (18+) who might require support at home as they leave hospital following a day procedure, an unexpected trip to A&E or after a more lengthy stay in hospital.
The service is inclusive for all and can run alongside other care/community packages which may also be helping people stay well and healthy at home. The Scarborough, Whitby and Ryedale Home from Hospital team are all passionate about helping clients overcome some of the difficulties on returning home.
We understand everyone is keen to get back home after a trip to hospital, but sometimes it’s the simple things that make this more difficult – like reaching everyday necessities around the house, taking the bins down to the road side, ensuring the essentials are in the cupboard, or navigating your new limitations (be them temporary or longer-term), our team want to ensure you can come home with the confidence you need to make the transition a success.
This free short-term support can build confidence and independence and gives peace of mind. The team will also signpost clients onto other appropriate support, services or eligible benefits. criteria. The following eligibility criteria applies to this service:
- Working full time
- Working part time
- Temporarily unemployed
- Other permanently unemployed
